Membership Form
All gymnastics are required to fill in an enrolment form on or before their first training session each year.
The information requested on the form includes emergency contact information and special health issues; to conform with the Club’s Health and Safety Policy it is imperative that forms are completed in full and returned to the office as soon as possible. It is essential that all fields are completed on this form.
